Community Consultation Service - Dual Diagnosis
The Community Consultation Service - Dual Diagnosis (CCS-DD) program is a specialized and time-limited Physician consultation service for individuals who have a mental health diagnosis, in addition to a diagnosis of intellectual/developmental disability and/or Autism Spectrum Disorder with significant adaptive/behavioural challenges. The main purpose of the CCS-DD program is to provide short-term comprehensive Physician consultation focusing on pharmacological management and creation of recommendations for ongoing support in the community, in collaboration with Primary Care Providers and other community-based organizations and support services (e.g., Developmental Services Ontario). Nursing support is also provided to monitor and support the implementation of recommendations and the treatment plan.
The hours of operation are Monday to Friday from 8 a.m. to 4 p.m. at the Community Health Hub at 287 Bayshore Drive, Midland. Services are also available virtually or by telephone.
Program Eligibility
Individuals who are 16 years of age and older that have a mental health diagnosis, in addition to a diagnosis of intellectual/developmental disability and/or Autism Spectrum Disorder and who are experiencing significant adaptive/behavioural challenges (e.g., aggression). This program is specifically for individuals with a dual diagnosis.
Referrals
Referrals to CCS-DD must be completed by a Primary Care Provider using the Waypoint Referral Form.
